The image you uploaded could not be read
Your documents are fine. The photograph of them was rejected. The upload must clearly show your printed name, full account number and IFSC in a single frame.
आपके कागज़ ठीक हैं, लेकिन उनकी तस्वीर साफ नहीं थी। एक ही फोटो में बैंक का छपा हुआ नाम, पूरा account number और IFSC साफ दिखना चाहिए।
Whose job it is to fix it
You
Nobody else is holding this up. It can be fixed from your side.
Raised at the Dealing Assistant / Social Security Assistant desk. Done correctly, this usually clears in about 3 days.
What to do, in order

Photograph the passbook's first page flat, in daylight, with no flash and no shadow across the digits.
पासबुक का पहला पृष्ठ समतल रखकर दिन के उजाले में, बिना फ़्लैश और बिना छाया के फ़ोटो लें।

Confirm your name is printed by the bank, not written by hand. If your cheque has no printed name, use the passbook instead.
सुनिश्चित करें कि नाम बैंक द्वारा छपा है, हाथ से लिखा नहीं। चेक पर छपा नाम न हो तो पासबुक का उपयोग करें।

Why the message could not tell you any of this
The upload is checked by a person, not a machine, and the file size ceiling pushes members to compress images until the digits blur. The most common failure is a handwritten name where the portal expects the bank's printed name — a cancelled cheque without a printed name is rejected however sharp the photograph.
Produced by an independent prototype of a provident fund member portal. Not an official EPFO document and not legal advice.
There is a letter for this
Each one is drafted with your own details and the correct statutory wording, ready to send yourself. No account, no sign-in, and nothing sent anywhere — they are assembled in your browser from eight things on the message that rejected you. Nothing is filed on your behalf.
- EPFiGMS grievanceRegisters the complaint inside EPFO and puts a dated reference number on it.
If nothing moves
The twenty-day settlement commitment restarts every time a desk returns the file, so it can be exceeded indefinitely without ever being formally breached. An RTI application about your own file must be answered in thirty days, and silence past that is a deemed refusal that opens an appeal — and a penalty on the officer personally.
Often the real problem instead
Your name is spelled differently in two places
Name in EPFO records not matching with Aadhaar
Your old employer never marked you as having left
Date of exit not updated by employer
Your bank account is not linked, or the name on it differs
Bank account not seeded with UAN
Your employer has not approved your KYC
KYC not approved by employer
